We had two in Ho Ho Homicidal and in both cases there was nothing that could have been done.
One involved an actor blowing off the shoot on Santa's last day. This however may be solved later with a minor reshoot.
The other involved a red t-shirt. We were losing access to the location and couldn't replace the shirt so we explained it with dialog. It turned out funny.
So when continuity issues happen what do you do?

once i was filming a skate video
and on of the skaters got whinny
so i booted his ass and replaced him with another kid
turned out the kid i replaced him with was alot better and was very good skater when the camera came out

Once I did a film for social Studies, a spoof of Mac and Pc commercials, comparing Norsemen and Anglo Saxons,
One of my actors was wearing a red shirt, and the next second it had a logo on it...

In a short film of mine there was an establishing shot where two closet doors behind my actors were open. But in the closeups they were closed!
Time explains that factor. They only began their conversation after a considerable amount of time which allowed them to close the doors.

I'm usually pretty good about continuity errors.
When they DO happen, I usually let them go, and the audience doesn't notice.
I have, however, changed the color of one actor's shirt in AE, but this was only once.

I've done that as well. A lot of times After Effects can help with something like that.

I've noticed that no one mentioned a person specifically for that job. We are trying to assemble our crew and continuity is one aspect we plan to have at least one person dedicated to. Plan to have some stills shot on site for quick review and maybe lessen the chance of that happening.

the official title is "Script Supervisor."
Anyway, while shooting our short film, we did one characters scene in my bedroom, and the next characters scene also in my bedroom. Turns out they were both supposed to be in different rooms, so we had to reshoot one of them. That was major continuity, but once, the character was wearing a sweatshirt, and in the next scene he wasn't, so we added a scene where he leaves his house and takes off his sweatshirt in between them.
You just have to think on your toes.

I've just changed a plot point within my series and I've got the choice of going back and changing a few little details, or somehow making a gag about the errors later.
I've decided to film an additional bit of dialogue explaining the continuity errors for the online version and I've fixed the problem for all subsequent releases.
Not a brilliant solution - but apart from wiping everyone's minds and bunging up a fresh version of certtain episodes I'm a bit stuck!
That's the trouble with discovering extra ideas as you go... happens when I do shorts too but at least I can reshoot bits in that case!
Glad to hear that you got a good gag out of the situation - sometimes the 'bad take' swings around and ends up being a great moment.

That's always a good crew position to fill. In our situation the shirt was 40 miles away. It was easier to make light of it with a script change.
